Issue Description:

Swift code compiling often crashes with multiple switch case fallthroughs and the same named associated values.

To reproduce the problem run (compile) the following code a few times:

enum Foo {
    case a(count: Int)
    case b(count: Int)
    case c(count: Int)
}

func bar(v: Foo) {
    switch v {
    case .a(let count): fallthrough
    case .b(let count): fallthrough
    case .c(let count):
        print(count)
    }
}

yes, SR-9698 apparently includes all the preconditions for the bug, described in my bug report, but mine is a more isolated case.
